A 58-year-old male presented with a challenging orthopedic scenario. He was an old operated case of partial patellectomy combined with a medial gastrocnemius flap, performed three months ago at another center. The patient required further intervention for patella tendon repair, which was successfully executed using advanced techniques, including poly-tape fixation.
Surgical Challenge
The case was particularly complex due to :-
➤ Previous surgical interventions leading to altered anatomy and scar tissue formation.
➤ Partial patellectomy affecting the biomechanical stability of the knee joint.
➤ Medial gastrocnemius flap reconstruction, which added another layer of complexity to the surgical field.
Surgical Approach
The procedure involved meticulous planning and execution :–
➤ Assessment :- The integrity of the residual patella, tendon, and surrounding structures was carefully evaluated.
➤ Poly-Tape Fixation :- A durable, flexible, and biocompatible poly-tape was employed to repair and reinforce the patella tendon. This technique offers superior mechanical strength and promotes early mobilization.
➤ Preservation and Integration :- Care was taken to integrate the repair with the previously reconstructed medial gastrocnemius flap.
➤ Rehabilitation :- A structured post-operative rehabilitation plan was outlined to ensure optimal recovery and functionality.
Outcome
The surgery was successful, and the patient is currently on a rehabilitation program aimed at restoring knee joint stability, range of motion, and strength. This advanced technique not only salvaged the compromised patella but also enhanced the structural integrity of the knee joint.
